Following a recent tender process the Northern Health and Social Care Trust has appointed Connected Health to provide a Rapid Response Domiciliary Care Service in both Ballymena and East Antrim Area.

The Rapid Response Service will provide short-term domiciliary care, for a period of up to 4 weeks. The Service User will then either no longer require care or will remain on the service for an extended period until a long-term service can be sourced.

By helping to facilitate people leaving hospital earlier frees up finite resources and allows the heath system to keep moving and help those in the most need of primary care.

Connected Health has been delivering domiciliary care in the community since 2006, Lorraine Corr, Service Manager said “We are delighted to have been awarded this significant contract, our team of carers provide a vital service to their local communities.”

The Rapid Response service provides an important route for patients to get out of hospital and back to their own environment. Clinical evidence shows that staying in a hospital bed when you don’t need to be there, is harmful for frail and elderly patients who quickly lose their physical strength and self-confidence. The sooner we can get people home to be as mobile and independent as possible, the better their mental and physical health is.”
