Connected Health is excited to announce a partnership with YOURmeds. The distribution agreement will bring innovation to medication management across Northern Ireland and Republic of Ireland.
YOURmeds smart medication management system brings medication adherence into the 21st century with it’s easy to understand, simple to use, portable medication management system. YOURmeds provides clinicians, adult social care teams and care providers with real time medication adherence data, enabling prescriptions and care packages to be adapted.
The system uses Strength Based Commissioning to increase independence and capacity within adult social care through digital rather than physical support. On average YOURmeds achieves an 84% adherence rate and from our data we know that 1 in 4 of our adherent rounds is due to a support intervention. Care Commissioners are seeing 4 times return on investment through a reduction in care hours provided.
